Description


This role is based at The Glen, but provides support across the Western PHN. The purpose of this role is to support clients when they apply for residential rehabilitation treatment, and while they are waiting for treatment. This means providing some basic case management, referring clients to appropriate services and coordinating their care.

Requirement


  • Relationship building skills are essential
  • Build trust with clients, community and service providers
  • Experience working in AOD sector (particularly residential rehabilitation) is desirable
  • Client-centred, culturally grounded and strength-based case management approaches
  • Ability to case manage yourself
  • Leadership & communication skills (written and verbal)
  • Be a great listener
  • Good computer & technology skills (including the ability to use and improve Client Management System)
  • Ability to provide solutions when you identify a problem.
  • Ability to plan your work, and manage your priorities, focus and time.
  • Ability to travel (inc. arranging travel and being flexible)
  • Understanding of risk.
  • Attention to detail.
  • Base decisions on facts.
